For the safety of your child and other students, it is the policy of Santiago Christian School Nurse’s Office that ALL medication be administered under the supervision of the nurse’s office. Students are not to carry medication with them at school. This is to protect students from possible misuse of medications which could have dangerous consequences. If your child needs to take a prescription during the school day I am asking that you bring the medicine to the nurse’s office in the morning with the medication check-in form and a copy of the doctor prescription. The medication form includes important information such as your child’s name, grade level, what needs to be given, at what time(s), any special instructions (i.e. medication needs to be refrigerated etc.), and your signature to authorize me to administer the medication. I will make sure that your child is given the medicine at the appropriate time. At the end of the school day, you may pick up the prescription from the nurse’s office before the child goes home. Please be aware that ALL MEDICATION to be given MUST come in the original bottle with a copy of the prescription. The nurse’s office will not administer any medication that is brought from home without a medication check-in form filled out and signed. This is to assist the nurse’s office to verify prescriptions and dosages to reduce the chance of error in administering medications.
If your child needs to take a long term prescription, for example Ritalin, or has a specific medication that is needed periodically such as migraine medication or asthma inhaler and is not stocked in the nurse’s office you may send a supply of the medication to the nurse’s office with a copy of the medication check in form; then when the supply is running low, I will send a note home to advise you to send in more. The medication will be labeled and stored securely specifically for your child. It will be administered to your child as prescribed. This will save you the hassle of needing to send it to school every day. Again, as with other medication, I will need a copy of the prescription to keep in your child’s Health file. It is also your responsibility to notify me of any changes in the dosage of your child’s prescription by bringing in an updated prescription. This serves to protect not only your child but myself as well.
Para la seguridad de su hijo y otros estudiantes, es la política de la enfermería de Santiago Christian School que TODOS los medicamentos se administren por la supervisión de la oficina de la enfermera. Nos aseguraremos de que su hijo tome la medicina en el momento oportuno. Al final de la jornada escolar, usted puede recoger la receta de la oficina de la enfermera antes de que el niño valla a su casa. Tenga en cuenta que TODOS LOS MEDICAMENTOSdeberán llegar en el frasco original con una copia de la receta. La oficina de la enfermera no administrara ningún medicamento que se traiga de casa sin un Formulario del Medicamentos lleno y firmado.
